Mobile Application Privacy Statement, v1, May 1, 2025

IndigeQuit is an application for smoking cessation developed at the Fred Hutchinson Cancer Center in Seattle, Washington ("We/Us", or "Fred Hutch"). At Fred Hutch we take your privacy seriously. We respectfully request that you review our Privacy Statement carefully so you understand our privacy practices. This Privacy Statement explains how We collect, use, disclose, and safeguard the information you provide to Us so you can make an informed decision when using IndigeQuit. This Privacy Statement covers both our website and mobile application (Service) solely for the clinical trials which require you to use our apps. By using our apps or otherwise providing your information to Us, you agree to the terms of this Privacy Statement. If you do not agree with the terms of this Privacy Statement, please stop using the apps.

INFORMATION WE COLLECT

We collect some non-personal information and some personally identifiable information (PII) when you access the mobile application. Non-personal information includes information that cannot be used to identify you, individually, like anonymous usage data, number of clicks that you make as you navigate the website or mobile application, etc. Personal Information includes information that can be used to identify you such as your email address. The collection of this information is regulated by Fred Hutch's Institutional Review Board.

HOW WE USE PERSONAL INFORMATION

When using our apps, We may ask you to provide Us with some personally identifiable information to create your User profile and to support your use of the apps. We describe how We use or disclose your personal information in more detail here:

User Information. The app includes an engaging and personalized experience for quit smoking program participants (Users). To effectively deliver our app to you, as one of our Users We may collect, store, and use personally identifiable information that you provided to Us when you registered. The use of this information is regulated by Fred Hutch's Institutional Review Board.

Communications. We may use personally identifiable information you provided to Us to respond to you, to provide what you requested, to update you about changes to our Privacy Statement or terms and conditions.

Research. We use identified information (information that is linked to your study identification number) to measure our apps’ impact and effectiveness to Users, and to conduct surveys and continue our research at the Fred Hutch, which may result in published reports or articles. De-identified information is used in our reports, research, surveys and results. In order to protect your privacy, We do not include any personally identifiable information in our research reports.

HOW WE PROTECT YOUR PERSONAL INFORMATION

When you share personally identifiable information with Us it is protected both online and offline. When We collect sensitive information like your name and contact information, the information is encrypted and transmitted in a secure manner. The computers and servers We use to store this information are kept in secure environments. Only employees and contractors of Fred Hutch with the need to access your information for the research study are granted access to your personally identifiable information.

ACCESS AND CONTROL TO YOUR PERSONAL IDENTIFIABLE INFORMATION

Your Rights. We are required by law to: (1) protect the privacy of your Personal Information, (2) provide you this notice explaining our responsibilities and privacy practices when We collect your Personal Information, (3) notify you of a breach of unsecured Personal Information, and (4) follow the terms of this Privacy Statement.

Data storage. We store Personal Information and other data for 5 years following the completion of the study, as approved by Fred Hutch's Institutional Review Board. After 5 years, all personal identifying information will be destroyed. All de-identified information will be retained for 10 years, and the retention period may be extended at Our request if approved by Fred Hutch's Institutional Review Board.

We will not share or sell your information (including your mobile number) to third parties.

MISCELLANEOUS PROVISIONS

Devices. We may collect information from or about the computers, phones, or other devices where you install or access the apps, depending on the permissions you've granted. We may associate the information We collect from your different devices, which helps Us provide consistent use of the apps across your devices. For example, connection information such as the name of your mobile operator or ISP, browser type, language and time zone, and IP address.

Cookies. We may use cookies and similar technologies to provide support of the apps.

Legal Proceedings. We may disclose your Personal Information in response to a court or administrative order, subpoena, discovery request, or other lawful processes.

UPDATES TO PRIVACY POLICY

We will notify you through our Website or Apps of any significant changes made to our privacy practices by updating our Privacy Statement, and include the opportunity to stop using the apps.
